postmarked no later than 07/30/09. 4. AcomData is not responsible for lost or misdirected mail. 5.
Limit ONE rebate per person, household, family or address. 6. This offer cannot be combined with
any other offer. 7. Requests from P.O. Boxes will not be accepted, and requests from invalid or
undeliverable mailing addresses will be rejected. 8. Offer limited to end-users only. 9. Requests for
